Even as access to vaccines increases throughout 2021, the need for COVID-19 testing is not going away. Our goal is to reach 4,000+ residents in the DC community, and to expand this sustainable model to address a multitude of health disparities in the Black community, such as high risks of diabetes and high blood pressure, in the post-pandemic future. On June 17, at our Juneteenth Town Hall event, United Way NCA, Choose Healthy Life and our partners made a big announcement on expanding the Choose Healthy Life programmatic work to include COVID-19 vaccinations and more. What is Choose Healthy Life?

Working in partnership with United Way of the National Capital Area, Quest DiagnosticsChoose Healthy Life Black Clergy Action Plan (CHL) is a response to the disproportionate impacts of the COVID-19 pandemic on the Black community. CHL leverages the power of the Black church—a trusted and respected organization in the Black community—to expand access to FREE COVID-19 testing, contact tracing, vaccines and other health services in underserved communities. CHL fosters partnerships between Black churches and Local United Way (LUW) affiliates to conduct a massive public health campaign across five cities: New York, Washington, DC, Newark, Atlanta and Detroit.

How to Get Tested and Vaccinated for COVID-19 at a CHL Site

All testing and vaccination sites through CHL are FREE for community members. Please go to Quest Diagnostics’ website to pre-register for your COVID-19 test and vaccine. Walk-ups are welcome and will be registered onsite, but pre-registration and scheduling ahead of time is highly recommended. Churches Serving as COVID-19 Testing and Vaccination Sites in the Washington, DC Area

In phase two of this initiative, there are a total of four church locations offering free coronavirus testing in Washington, DC, Montgomery County and Prince George’s County and participating in the Choose Healthy Life (CHL) initiative. Each church has hired a Public Health Navigator, establishing an infrastructure of trusted health support to ensure testing is safe, effective and reliable. Our partner churches are listed below.

Get a Free Lyft Ride to Your COVID-19 Vaccine

Our Ride United program is offering free Lyft rides of up to $25 each way for those traveling to their COVID-19 vaccine appointments.

A person is eligible to receive this service if they:

  • Have a vaccine appointment scheduled
  • Have no other transportation options available (via Medicare/Medicaid)
  • Live in an area where Lyft is available
  • Are 18 years old or older, or can be accompanied by a guardian
  • Have a smartphone with the Lyft app to receive the discount codes

If you are eligible, give us a call to get your code for your free Lyft ride at 202-488-2008. Phone lines are open Monday through Friday, from 9 a.m. to 5 p.m.

During their ride, clients must wear face coverings, leave the front passenger seat empty and put the windows down when possible.

Choose Healthy Life is raising awareness and educating the Black community on COVID-19 and other health disparities. CHL proactively engages the community in a COVID-19 testing and vaccination campaign to reduce the spread of the virus and to save lives.

Who should be tested for COVID-19?

The CDC recommends that anyone with any signs or symptoms of COVID-19 get tested, regardless of vaccination status or prior infection. If you have also been exposed to COVID-19, testing is highly recommended.

Who should get the COVID-19 Vaccine?

COVID-19 vaccination is recommended for all people aged 12 years and older, including people who are pregnant, lactating, trying to get pregnant now or might become pregnant in the future. If you are concerned about the vaccine, please speak with your doctor or a health expert available at all Choose Healthy Life events.

Which vaccine does the Choose Healthy Life program provide?

Our vaccination events primarily provide the Pfizer-BioNTech vaccine as it is fully FDA approved and the only vaccine approved for those ages 12 and older. However, depending on the location, supply and needs of the community, any of the three major COVID-19 vaccines (Pfizer, Moderna or J&J) may be used. For example, if a population has trouble coming in for a second dose, then we may administer a single dose J&J vaccine. Additionally, if a booster dose is approved, CHL locations will likely have some stock of all three vaccines.

Do I need to bring insurance information to the testing and vaccination site?

Insurance is not required for testing or vaccination and all testing and vaccination is free to community members.

I don’t drive. Can I walk in?

All of our testing and and vaccination events are walk-up events and do not have drive-thrus. Walk-in appointments are available for all community members, but pre-registration is recommended. You can pre-register here for a COVID-19 test or vaccine.
